This Privacy Policy (“Policy”) sets out how we, Medical Transportation Management, Inc., our affiliate companies and subsidiaries (collectively, “MTM”), collect and use your personal information when you use the MTM websites. By using any of the MTM websites, you consent to the collection and use of your personal information as described in this Policy.

Collection of Your Personal Information

We collect such personal information including but not limited to your:

  • E-mail address;
  • First and last name;
  • Telephone number (mobile and home);
  • Date of birth;
  • Medicaid ID;
  • Mailing addresses;
  • Disability information for ADA assessments;
  • Driver’s license/insurance/vehicle information for transportation reimbursement.

We also automatically collect information about your computer hardware and software, such as your IP address, browser type, domain names, access times, referring website addresses, and pages you visit. We use this information to maintain quality of our services and to provide general statistics regarding use of our websites.

Most of the information listed above will constitute “Personally Identifiable Information” or “Personal Data” depending on the extent to which such information is protected by relevant privacy or data protection laws.

Use of Your Personal Information

We will only use your personal information in accordance with applicable law and for the following purposes:

  • To operate our websites and deliver the services you have requested, including without limitation, any interactive services you use;
  • Improve existing or develop new services;

Use of Cookies and “Do Not Track”

We use “cookies” on this site. A cookie is a piece of data stored on a site visitor’s hard drive to help us improve your access to our websites and identify repeat visitors to our sites. For instance, when we use a cookie to identify you, you would not have to log in a password more than once, thereby saving time while on our site. Cookies can also enable us to track and target the interests of our users to enhance the experience on our site. Cookies do not damage your computer, and usage of a cookie is in no way linked to any personally identifiable information on our site. Most web browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer.

California law requires us to let you know how we respond to web browser “Do Not Track” (DNT) signals. Because there currently is not an industry or legal standard for recognizing or honoring DNT signals, we do not respond to them at this time.


We respect the privacy and confidentiality of your personal information. As such, we have implemented technical and organizational measures to protect your personal information in accordance with industry standards. When you submit sensitive information via any of the MTM websites, your personal information is protected both online and offline. Wherever we collect such information, the information is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for “https” at the beginning of the address for any of our websites. The computers/servers in which we store personal information are kept in a secure environment.

While we use encryption to protect your personal information transmitted online, we also protect your information offline. Only employees, contractors, and agents who need the information to perform a specific job or function are granted access to your personal information, and such individuals are subject to strict contractual confidentiality obligations. We also review our information collection, storage, and processing practices to prevent unauthorized access to our systems and to make improvements when and whenever necessary to ensure continued protection of your information.

Ultimately, however, it is important you understand that no security measure is completely secure or infallible (e.g., from malicious intrusion), and we cannot guarantee complete security of your personal information in every instance.

Disclosure of Personal Information

We may disclose your personal information to third parties, but only in accordance with applicable law and for the purposes described in this Policy. These third parties may include:

  • Our affiliates or subsidiaries;
  • Our outsourced or contracted providers or vendors for the purpose of providing our services;

Retention of Personal Information

We will retain your personal information for as long as is reasonably necessary for the purposes listed herein or as required by applicable law.


MTM monitors network traffic, web and application servers, and logs to identify unauthorized attempts to upload, change or otherwise cause damage to our websites and data. If any unauthorized access to or use of Personally Identifiable Information occurs, despite the protections established within this Privacy Policy, MTM will notify you of a breach in security of the system following discovery in one or more of the following ways: email, mail, posting of the breach on the MTM website, or by notifying the media (if applicable).


The MTM Websites and MTM do no knowingly solicit data from children, and we do not knowingly market to children. Specifically, in accordance with California’s Children’s Online Privacy Protection Act (COPPA), the MTM Websites are general audience websites and are not directed to children under the age of 13 and do not knowingly collect information from children under the age of 13.

Inclusion of California-Specific Provisions

The California Consumer Privacy Act (CCPA) provides California residents with specific rights with respect to their personal information. Under the CCPA, California residents have the right to:

  • Know whether their personal information is sold or disclosed and to whom;
  • Say no to the sale of their personal information;
  • Access their personal information;
  • Unless prohibited by applicable law, have their personal information deleted; and
  • Have the right to equal service and price, even if they exercise their privacy rights under this law.
